There are three principal ways that Association Home Care obtains personal information directly from you:

(1) via your filling out and submission of an application for prospective employment including caregiver;

(2) from your inquiry into locations and services.

The information requested in the Association Home Care online application for prospective employment including caregiver may include:

your name,

address,

telephone number,

email address,

date of birth (age),

social security number,

driver’s license number,

employment information,

former resident information,

background information,

in addition to other information.

If you voluntarily submit your information, Association Home Care will use it to fulfill your request, inform you about new job opportunities or available services, and support other business-related activities. If you apply for a caregiver position, Association Home Care may conduct a background check and share relevant information with a third-party reporting agency as part of that process.

All emails sent by Association Home Care will include instructions on how to unsubscribe from the mailing list, whether you are a client or a caregiver applicant.

A1. Personal Information

Association Home Care may use service providers to operate the Site and employ other third parties to perform work on its behalf, such as the web hosting of the Site. These parties may have access to personal information that you submit through the Site, but only for the purpose of performing their duties in connection with the Site.

Additionally, Association Home Care’s affiliates, Google and other third parties that you authorize Association Home Care to disclose your personal information to in connection with services, products, or potential employment (Caregiver), may have access to your personal information. B. Non- Personal Information

The Site collects and stores information each time a user interacts with it. Association Home Care automatically logs this data on its servers. Even if you do not submit personal information, Association Home Care—through tools such as Google Analytics™, Google Ads™, Google Forms™, Google Maps™, and IP lookup services—actively gathers details about your interaction with the Site.
